Background: There is growing evidence that exercise have positive effects on Mild Cognitive Impairment (MCI). Exer gaming is the combination of physical activity and cognitive training, their combined effect could cause changes then single therapy in mild cognitive impaired (MCI) patients.

Purpose: The objective of this study was to compare the short and long term effects of Xbox 360 kinect cognitive games and aerobic exercise on electroencephalogram parameters and cognition of MCI patients

Methods: A parallel group design study was conducted in Pakistan Railways General Hospital, Rawalpindi. Informed consent was taken from all the patients. Total 84 patients were selected and randomly divided into Xbox 360 Kinect cognitive games group (n=22) aerobic exercise group (n=20) and control groups. The outcome measures were Mini mental state examination (MMSE), Montreal cognitive assessment scale (MoCA), trail making test (TMT) A and B, slowness and complexity of EEG. EEG was recorded using the EMOTIV EPOC with 14 channels. Patients were assessed before and after one session of intervention for short-term effects and after six weeks for long-term effects.

Results: After one session of intervention delta waves significantly improved (p 0.05) in Xbox 360 Kinect group as compared to aerobic exercise group. Whereas after six weeks of intervention there was no significant difference (p 0.05) in both groups. Complexity or approximate entropy (ApEn) of EEG revealed that there was significant improvement (p 0.01) after six weeks treatment of aerobics exercise group as compared to Xbox 360 Kinect games group. The neuro-cognitive tests results showed that there were significant improvement after six weeks in MMSE, MoCA, of aerobic exercise group as compared to the Xbox 360 kinect cognitive games.

Conclusion(s): Aerobic exercise seems to be have more short and long-term effects on MCI patients as compared to Xbox 360 Kinect based cognitive games. These exercises help in to improve cognitive plasticity in MCI patients.

Implications: This study highlighted the effects of physical activity of MCI patients. The early stage of cognitive impairment could be better stage to intervene by physical activity. It will also improve physical therapy patient care of cognitively impaired patients.
