DOES PHYSICAL THERAPY HAVE A ROLE IN MANAGING MASSIVE CAUSALITIES?

Background: Gaza strip consider one of the most trigger area in the world due to ongoing conflict, this conflict cast a shadow on all life aspect mainly the health sectors which already fragile due to chronic scarcity of resources.
Since march 2018 Palestinian people in Gaza strip demonstrate on the buffer zone on the boarder which result in more than 13,000 injured.
Gaza strip have five main hospital, the major one is Al Shifa medical complex which located in Gaza city, the hospital received 70% of the total cases.

Purpose: The main purpose is to:
1- Investigate the nature of role the physiotherapist can play in massive causalities.
2- To raise up the awareness of medical staff on the role of physiotherapists in emergency.

Methods: The starting point was the risk management report published in WCPT website which used to convincing the decision maker in MOH to engage physiotherapist in emergency team.
After that, physiotherapy department formulate an emergency plan, this plan shared with related parties as it is the first experience , the plan was approved and completely supported by hospital manager and physiotherapy rehabilitation unit in MOH .
The next step was formation of emergency team with based on tight criteria include:
1- Welling to work on emergency department
2- 10 years experience or above
3- Advanced courses in musculoskeletal assessment and management
4- Good communication skills with medical staff.
Five physiotherapists was selected, one of them was assigned as a leader.
After that , the team design forms for documentation purposes, the forms then accredited by the MOH and disseminated for all the hospitals.
For the first time in MENA, Physiotherapy emergency team work on emergency department for about 5 months, the team was in the first contact with injured, the team deal with thousands of injured with massive, multiple and complex trauma.
The intervention include managing and controlling triage zone completely, doing clinical assessment for musculoskeletal trauma, requesting x ray and lab. investigation, education the patient and guiding them for service provider after discharge, dressing, psychological support and writing discharge sheet.

Results: The team deal directly with more than 63 % of the cases
- Overload upon the medical staff decreased by 33%
- length of stay of patient on ED reduce significantly.

Conclusion(s): Physical therapy have a crucial role in managing massive causalities which will improve the efficiency of the emergency system and decrease disability rate and minimize complications of the risk.

Implications: The success of this story will significantly raise the position of the profession nationally and will give a chance to engage PT in emergency department pertinently also it will open a window for us to share our experience outdoor and networking with PT international bodies.
