PELVIC FLOOR TREATMENT OF THE NEUROLOGICALLY INVOLVED PATIENT

Background: Patients that have neurologic deficits often have a variety of bowel and bladder concerns that can severely affect their quality of life. Many of these concerns are overlooked by their physicians due to the multiple other medical concerns that are being managed. Multiple studies of patients with MS, SCI and CVA will point to the fact that bowel and bladder concerns can be the largest impact on quality of life for these patients.

Purpose: The purpose of this talk will be to share a screening program for physiotherapists to use with patients with neurologic deficits to look for bowel and bladder concerns and facilitate an appropriate pelvic floor physiotherapy referral. The speaker has instituted a treatment algorithm for screening and treating patients for bowel and bladder concerns as well as both a consultation model and traditional treatment model for pelvic floor therapy. Diagnoses discussed will include multiple sclerosis, spinal cord injury, CVA, and Parkinson´s disease. Current research will be briefly touched on for both traditional medical interventions as well as appropriate physiotherapy interventions for this population. Case studies will be highlighted for patients from a multihospital neuroscience program.

Methods: NA- this is not a research study, it is a presentation of a screening program and treatment pathway.

Results: Patients that are at least able to consult with a pelvic floor physiotherapist have higher overall treatment satisfaction scores when being treated for a neurologic problem. By providing a treatment pathway for these patients they feel that all of their needs are being addressed and they have higher overall quality of life scores.

Conclusion(s): Patients that are being treated for acute neurologic insults should be screened and possibly treated by a pelvic floor physiotherapist during their time at the outpatient rehabilitation facility.
