PREDICTORS OF FALL DURING THE FIRST YEAR AFTER TOTAL HIP ARTHROPLASTY (THA).

Background: Fall after total hip arthroplasty (THA) causes severe complications such as dislocation and fractures around the stem. Therefore, the investigation about preventing falls after THA is important.

Purpose: The purpose of this study was to investigate factors predicting a fall during the first year after THA.

Methods: This study was a prospective study. The subjects were 127 patients with unilateral hip osteoarthritis (OA) who have no history of falling within one year before surgery (Age 63.5±9.7 years, 11.0% M, 89.0% F, BMI 23.3±3.4 kg/m2). We investigated whether there was a fall during the first year after THA and divided into the two groups: fallers and non-fallers. Functional performance (hip abductor muscle strength on the affected side (HA), one leg stance time on the affected side, maximum walking speed (MWS)), pain during walking, and physical activity were assessed preoperatively to predict a fall.

Results: Of the 127 subjects, 123 subjects (96.9%) who followed up until one year postoperatively were analyzed. Of these, 20 subjects (16.0%) had experienced falls. In particular, Fallers were significant lower HA compared to non-fallers (Fallers: 0.48±0.19 Nm/kg, non-fallers: 0.61±0.22 Nm/kg, p 0.05). Multiple logistic regression analysis revealed that HA (Odds ratio (OR):1.05, 95% Confidence Interval (CI) :1.01-1.08, p 0.01) and MWS (OR:0.17, 95% CI:0.03-0.97, p 0.05) were significantly related to falling.

Conclusion(s): This study showed that preoperative HA and MWS were predictors of fall occurring within the first year after THA. Therefore, it is necessary to improve preoperative HA and MWS in terms of prevention of falls within the first year after THA.

Implications: Rehabilitation programs to improve preoperative HA and MWS are necessary to prevent falls during the first year after THA.
