The relation between quality of skeletal muscle and physical function in patients with leukemia: suggestion for an alternative assessment

Purpose:

The purposes of this study were to investigate the relation between physical function and quality of lower limb muscles and to compare psoas with other muscles.The purposes of this study were to investigate the relation between physical function and quality of lower limb muscles and to compare psoas with other muscles.

Methods:

This cross-sectional study was conducted on adult patients diagnosed with AML. CT values (CTVs), used as the indicator of the muscle quality, were measured from CT images of psoas, rectus femoris, gluteus maximus (GMax) and gluteus medius (GMed) at the time of diagnosis. Physical function was evaluated with Cancer Functional Assessment Set (cFAS) which is used as an assessment tool for physical function specific to cancer patients. Multiple regression analysis was carried out to investigate the relation between physical function and CTVs of each muscle adjusted for age, sex and BMI. In addition, independent factors related to cFAS significantly were compared by their effect sizes. Statistical significance was set at 0.05.

Results:

A total of 51 patients (31 men, mean age of 65.5±15.8 years) were included in this study. There were significant relations between cFAS and CTVs of psoas, GMax and GMed (β=0.57 adjusted R2=0.19, β=0.54 adjusted R2=0.23, β=0.84 adjusted R2=0.31, respectively). The effect size of the regression equation using GMax and GMed was larger than that of psoas.

Conclusion(s):

The high quality of skeletal muscle related to physical function effectively. GMax and GMed were considered the predictors of physical function as psoas was, and these muscles have the promise of better predictors of physical function.

Implications:

It is important to evaluate the muscle quality of patients with AML properly. In addition to psoas, GMax and GMed could be worth treating as predictors of physical function. We expect this suggestion will help in the appropriate assessment for the quality of skeletal muscle.
